Registration Requirements

To be enrolled in kindergarten in August, a child must have their fifth (5th) birthday no later than September 2 of the year entering school. To be enrolled in first grade, a child must have their sixth (6th) birthday no later than December 31 of the year entering school. All children must live within the city limits of Opelika and within the school zone in which they are registering.

WHAT YOU WILL NEED:

1. BIRTH CERTIFICATE (state issued) or other documentation establishing date of birth. Examples include: A religious, hospital, or physician's certificate showing date of
birth: a baptismal certificate; an entry in a family bible; an adoption record or previously
verified school records.
Directions for acquiring a birth certificate:

a) ALABAMA: Get form and assistance to complete from the county
health department. Certificates will be issued to immediate family members only.

b) OUTSIDE ALABAMA: Request certificate from the Bureau of Vital Statistics in the
state where the child was born. Along with the application, send name of child,
name of father, maiden name of mother, and date and place of child's birth.

2. CERTIFICATE OF IMMUNIZATION FORM
Bring the child's State of Alabama Certificate of Immunization.

To receive updated immunizations, you will need to make an appointment for your
child at the Lee County Health Department (745-5765) or with your child's physician.
It is recommended that appointments be made as soon as possible to avoid delays in
completing school registration.

3. CHILD'S SOCIAL SECURITY CARD (requested but voluntary)
Social Security cards are used as a unique form of identification for students. Providing
this piece of identification is strictly optional. The number is not printed on any form or
report except as required by college on transcripts. If a social security number is not
provided, a temporary number will be assigned as per the State Department of
Education.

4. PHOTO ID

Parent/Guardian must present a photo ID (driver's license, non-driver identification, military ID, work ID, etc.) to determine custodial relationship of child. (Not to determine residency)

5. TWO PROOFS OF RESIDENCY

Parent/Guardian must present two proofs of residency:

  Primary Source-current power bill

  Secondary Source-current water, gas, trash, cell phone bill, lease, or mortgage deed.

6. CUSTODIAL DOCUMENTATION (if applicable)

Any applicable divorce decree or court-ordered custody documents.

7. PROOF OF GRADE

Withdrawal documentation and report showing current grades from previous school.
